What Insurances Are Accepted?
We accept all major PPOs and Medicare. However we do NOT accept HMOs or Medi-Cal. For patients without coverage, we offer consultation on a cash fee-basis. Please contact our office for coverage-related questions.
What do I need to bring for my first visit?
- Identification (Driver License or Passport)
- Insurance Card (digital or physical)
- Fill out New Patient Paperwork (DOWNLOAD LINK)
- If you don’t have a printer, please arrive 15 minutes early to your appointment to fill it out in our office
- Previous records (Imagings Reports – Xray, CT, MRI along with the physical discs if you have them ; Laboratory Studies ; previous consultation notes)
- If you are on chronic pain medication, we require the last 2 progress notes from your previous pain management provider
- Records can sent ahead of time via email or fax: (949) 736-5997
How Do I Get the Most out of my First Consultation?
Please arrive 15 minutes prior to your scheduled appointment time. We understand that unexpected circumstances may arise that cause delays. Otherwise, if you arrive more than 30 minutes after your appointment time, we may have to reschedule out of courtesy to our other clients. Please contact our office so that we can make every effort to accommodate you.

Do you take walk-in?
We strive to provide the highest quality of care, as such patients are only seen on a scheduled basis. Often, we can accommodate new patients within the same week for initial consultation. To schedule an appointment and start your journey towards pain relief, please contact our office today.
Are procedures done the same day as the consultation?
At your initial consultation, we will perform a thorough evaluation and assessment to determine the most suitable procedure to perform. Then we will check with your insurance regarding prior authorization requirements for the procedure. We will always try our best to expedite your care.
